According to the wiki, the relic itself is a Tides of the Foscari DLC addition. It shipped with patch v1.4.200 on 1 June 2023 as part of the "Bugfixes & tweaks" update that polished the early Tides DLC content. Internal ID is RELIC_MAP_FS.
The Milky Way Map gate is the most-missed detail. Per the wiki, the Lake Foscari Map "will not function" until the Milky Way Map is also collected. The Lake Foscari Map can be picked up first, but its pause-screen overlay stays disabled until the global Milky Way Map prerequisite clears.

According to the wiki, the relic location overlaps with the Yellow Sign Metaglio Right route. A single northeast-of-spawn detour can grab both pickups on the same trip. In real runs, this saves the second hidden-area detour and frees up minutes for the Seal of the Lake destroy sequence Keitha's build targets.
The relic sits inside Lake Foscari, the first stage of the Tides of the Foscari DLC. Per the wiki description, Lake Foscari is "an enchanted forest stuffed with fae mysteries and mythological entities." The stage unlocks immediately on DLC purchase.
