How Pocket ICBM Works
First stack: every missile fires three instead of one. The +2 additional missiles applies to each missile in a given volley. An AtG Missile Mk. 1 that normally fires 1 missile fires 3. A Disposable Missile Launcher that fires 12 missiles now fires 36. The tripling is per individual missile, not per activation.
Additional stacks do NOT add more missiles. A common misconception. Stacks 2, 3, 4+ each add +50% TOTAL damage to all missiles - they do not fire 5, 7, or 9 missiles. The missile count is locked at triple from the first stack onward. If you already have 1 Pocket ICBM, further stacks are a damage bonus only.
The damage bonus is TOTAL damage, not base damage. This matters for how procs interact. +50% TOTAL damage is applied after all other multipliers, meaning it does not scale with proc coefficients or other damage multipliers in the same way base damage items do. It directly multiplies the final missile damage value.
Each of the three missiles can proc on-hit effects independently. The extra two missiles from Pocket ICBM each have the same proc coefficient as the original missile. This means 3x the proc rolls, 3x the AtG triggers, 3x the Ukulele chances, and 3x the bleed applications per missile event. The proc chain potential is enormous.
Spare Drone Parts rockets are NOT affected. Despite looking like missiles, Spare Drone Parts' Col. Droneman rockets are not classified as the missile type that Pocket ICBM interacts with. Confirmed via in-game testing. Similarly, N'kuhana's Opinion skulls and Little Disciple wisps are not affected.

Still 3 missiles. Each missile now deals 50% more TOTAL damage. An AtG proc goes from 3x300% to 3x450% total damage.
3 Stacks
3x missiles, +100% dmg
Still 3 missiles. Each missile deals double the base missile damage. AtG proc: 3x600% total damage per trigger.
The first stack is by far the most valuable. Tripling missile fire count is more impactful than +50% damage on missiles. After 1 ICBM, additional stacks compete with other red items for the damage bonus only. In most runs, 1 ICBM is the correct target - not multiple.

Thermal Harpoons are classified as missiles. Each harpoon fires 3 with ICBM. Engineer also benefits from AtG through turrets, and each turret inherits ICBM - meaning both turrets fire tripled missiles simultaneously. One of the highest missile density survivors in the game.

S Tier
Fastest attack speed in the game means AtG and Plasma Shrimp procs fire constantly. With ICBM each proc becomes 3 missiles. Huntress turns into a missile barrage that can clear rooms without any direct combat. Best survivor for a Plasma Shrimp + ICBM + Transcendence build.
Commando
A Tier
Suppressive Fire hits many times per cast at 1.0 proc coefficient, triggering AtG constantly. Each AtG trigger becomes 3 missiles. Commando stacks missile procs naturally through sustained rapid fire. ICBM turns every Suppressive Fire volley into a missile storm.
Artificer
A Tier
Flamethrower fires at high proc rate and each tick can trigger AtG. With ICBM and Plasma Shrimp + Transcendence, Artificer fires 3 missiles per Flamethrower tick. Combine with Brainstalks for near-infinite Flamethrower during elite fights.
Key Synergies

Plasma Shrimp + Transcendence + ICBM is the defining missile build. Transcendence converts all HP to shields - permanently satisfying Plasma Shrimp's shield requirement. Every hit fires a missile. ICBM triples that to 3 missiles per hit. On a high-attack-speed survivor like Huntress, this produces hundreds of missiles per second in dense combat. One of the strongest raw DPS builds in the game.
Disposable Missile Launcher + Gesture of the Drowned + ICBM = 36-missile burst on cooldown. Gesture halves equipment cooldown and auto-fires it. ICBM triples every missile in the 12-missile volley to 36. With Soulbound Catalyst reducing the cooldown further on kills, the Launcher fires multiple times per stage event.
57 Leaf Clover increases AtG proc rate, feeding more ICBM triggers. AtG Missile Mk. 1 has a 10% proc chance. Clover re-rolls failures, pushing effective proc rate to ~19% at 1 Clover. More AtG procs = more triple-missile bursts per second. The two items compound: Clover generates the procs, ICBM multiplies the output of each one.
Fuel Cell extra charges let you bank multiple Disposable Missile Launcher volleys. Fuel Cell adds additional equipment charges. With 3 Fuel Cells and a Gesture, you can store 3 charges of the Launcher and discharge 108 missiles in rapid succession. A single Teleporter boss fight can be ended instantly by detonating stored charges.
Each extra missile has its own proc coefficient - this matters for proc chains. The 2 bonus missiles from ICBM each carry the same proc coefficient as the original missile. An AtG missile at 1.0 proc coefficient generates 3 independent proc rolls on hit. Against a pack of enemies each missile can independently trigger Ukulele, bleed, or further AtG procs - rapidly cascading into a full proc chain.
