Blacklisted items, on-kill items, sprint items, equipment-related items, boss items, and equipment are all excluded. Lunar items are also excluded normally - but Eulogy Zero (SotV DLC) can replace any granted item with a lunar one at 5% chance per stack.
Choose every item to specifically counter the evolved monster pool. Stack Old Guillotine, Brainstalks, and APR to handle increasingly dangerous enemies.
Itemized monsters drop items on death. By Stage 4 each evolved kill drops a higher-value item (monster has uncommons and legendaries). High risk, high reward.
All monsters are elites AND gaining items each stage. By Stage 3 you face itemized elites with Malachite shields or Glacial freezes. Very high difficulty.
Evolved monsters kill each other with their proc items. Gasoline, AtG, and band procs from itemized enemies deal friendly fire. Chaotic self-destruction runs.
Pure challenge modifier. Stage 1 and 2 feel normal. Stage 3 gets harder. Stage 4 is brutal. Stage 5 with 2 uncommons and 1 legendary on every enemy is a serious endgame challenge.
5x damage cuts through even heavily itemized monsters. But they hit back harder too. At 10% HP, a Lemurian with AtG Missiles can one-shot you. Very high skill requirement.

Not quite. Monsters spawned by a Combat Director on team Monster gain items. Void enemies spawned by the Camp Director (inside Void Seeds) do not - they spawn on team Void. However, Void Reavers spawned during normal gameplay and Void Invasions DO gain items since those are spawned by Monster-team Directors. Newt and the Artifact Reliquary do not gain items.
