The Artifact of Frailty is one of only two artifacts in the game with a code split across two separate stages. The full pattern only becomes clear once you've seen both halves. The other half in Void Fields requires taking the blue orb portal in the Bazaar Between Time.

10% HP means even a minor fall kills you. Doubled lethal fall damage at 10% health is instant death from any height. Possibly the most punishing artifact combination.
Enemies drop items without chests. Stage economy stays normal but the cliff danger is constant. Frailty adds tension to stages like Siren's Call where exploration risks falls.
Choose H3AD-5T v2 and Hopoo Feathers early to neutralize the artifact completely. With Command, Frailty becomes a cosmetic modifier with no real risk.
Moderate difficulty increase. Careful play eliminates most risk. Loader removes it entirely. H3AD-5T v2 turns it into an advantage. Weakest difficulty modifier in the set.

Yes. Loader's passive ability grants complete immunity to all fall damage, and this immunity applies to the Artifact of Frailty. Running Frailty as Loader is entirely consequence-free - it has zero effect on her gameplay. This makes Loader the best survivor for any run that includes Frailty.
H3AD-5T v2 negates the fall damage from Frailty by converting it into the item's kinetic ground explosion. Instead of taking doubled lethal fall damage, landing with H3AD-5T v2 creates the slam AoE and deals no damage to you. It is the only item that directly counters the artifact - and with Frailty active the slam explosion still scales with fall distance.

The trial for Frailty takes place in Bulwark's Ambry with Frailty active. The Ambry arena has block-shaped platforms and gaps between them. Falling off a platform during the trial deals doubled lethal fall damage. The Ambry trial is one of the more dangerous ones for this artifact since the arena layout creates regular fall opportunities during combat.
