Reach Sky Meadow (Stage 5) or Helminth Hatchery (SotS DLC). Drop through one of the two floor doors to reach the Compound Generator below the island.
Use the 9 switches on the Compound Generator to set your 3x3 pattern. Each switch cycles through: ■ Square → ● Circle → ▲ Triangle → ♦ Diamond.
Interact with the laptop next to the generator to submit the code. If correct, a red Artifact Portal spawns. You can skip the teleporter event entirely and go straight through the portal.
The portal leads to Bulwark's Ambry. Kill enemies to collect Artifact Keys. Use keys one at a time on the Artifact Reliquary. Destroy it to unlock the artifact.
After entering a valid code at the Compound Generator, the Artifact Portal appears immediately. You can go straight through it without activating or completing the teleporter event. This skips the rest of the stage.
When you use an Artifact Key on the Reliquary, ALL keys in your inventory are consumed at once. Pick up one key, use it, then pick up the next. Never carry more than one at a time or you waste them.

No. You can enter the Compound Generator and go through the Artifact Portal before activating the teleporter. Going through the portal skips the rest of the stage entirely.
There are four symbols: Square (■), Circle (●), Triangle (▲), and Diamond (♦). Each switch on the Compound Generator cycles through them in that order. The 9 switches form a 3x3 grid matching the pattern on each artifact's stone tablet.
After entering a correct code and going through the portal, you arrive in Bulwark's Ambry. An Artifact Key spawns in front of the Artifact Reliquary. Kill enemies to get more keys. Use one key at a time on the Reliquary - using a key consumes all keys in your inventory at once, so don't stockpile them. Destroy the Reliquary to unlock the artifact.
Artifact of Rebirth has no code. You must defeat the False Son boss (SotS DLC) and then interact with the Shrine of Rebirth that appears after the fight. Offer an item to it. This completes the Experienced Rebirth challenge and unlocks the artifact.
Yes. You can enable any combination of unlocked artifacts at the start of a new run from the lobby screen. In multiplayer, any artifact unlocked by any player in the lobby is available to the whole team.
Each artifact's code is engraved on a stone tablet hidden somewhere in the game world. For example, Command is in the Bazaar Behind Time, Sacrifice is at the Gilded Coast, and Vengeance is in A Moment, Fractured. The quick reference table above lists every location.
