Yellow items (also called boss items or planet items) are a special rarity in Risk of Rain 2. They cannot drop from chests. Most have a 15% chance to replace the Uncommon teleporter reward when their specific boss spawns as the Teleporter Boss. There are 22 yellow items total - 15 from specific bosses and 7 from special sources.

How the 15% Drop Chance Works
After completing a Teleporter Event, each Uncommon item that would drop has an independent 15% chance to be replaced by the boss's corresponding item. If the Teleporter Boss had no associated boss item (Scavenger, Horde of Many), all drops will be Uncommon. Shrine of the Mountain adds more item drops, each with its own 15% roll.

How to Farm Yellow Items
Shrine of the Mountain is the best farming tool. Each activation adds more boss spawns and more item drops - each drop has its own 15% chance to be a boss item. Multiple Shrines on one stage can produce several boss items from a single event.
Artifact of Command lets you choose any yellow item freely from a Command Essence. When a boss item would drop, you select it from the pool instead of getting a random one.
57 Leaf Clover improves the 15% drop roll, making boss items more likely to appear when their boss is the Teleporter Boss.
Yellow 3D Printers let you convert Yellow Scrap (from Scrapping unwanted boss items) into the specific item shown on the printer. A targeted way to convert unwanted duplicates.
Looping increases the chance of rarer bosses appearing (Overloading Worm, Magma Worm) which only spawn reliably post-loop. Charged Perforator specifically requires looping to appear at all.
