Aegis converts barrier into shield but Knurl regen refills HP below the barrier. Aegis generates barrier from healing. Titanic Knurl's passive regen counts as healing. On looping runs with high regen and high HP, the combination means you frequently generate small amounts of barrier from regen. More useful on survivors who already stack HP-heavy builds.

Titanic Knurl raises your HP and lets you regenerate that reduced damage back between engagements. Tougher Times blocks damage entirely, letting Knurl regen have more impact per second. A blocked hit is a hit that does not deplete any HP, allowing the constant regen to stay ahead of incoming damage more reliably. Yes. Unlike Cautious Slug which only regenerates outside of danger, Titanic Knurl regen is active at all times - in combat, during teleporter events, during boss fights. It is always running in the background regardless of what is happening on screen.
The regen bonus increases by 0.32 HP/s for every player level per stack. The formula is: regen per stack = 1.6 + (0.32 x player level). At level 10 each Knurl provides 4.8 HP/s. At level 20, 8.0 HP/s. At level 30, 11.2 HP/s. The in-game tooltip only shows the base 1.6 and does not reflect this scaling at all, which is why many players underestimate the item on longer runs.
Titanic Knurl drops from Stone Titan teleporter bosses. The drop is not guaranteed - Stone Titan must appear as the teleporter boss and then drop the Knurl rather than another boss item. Trophy Hunter's Tricorn (SotV DLC) can execute a Stone Titan for a guaranteed drop. Shrine of the Mountain increases boss spawn count, giving more drop chances. 57 Leaf Clover improves boss item drop luck.
Yes. Rejuvenation Rack doubles the strength of all healing including base health regeneration. Each Titanic Knurl's regen contribution is doubled by Rejuvenation Rack. With one Rack at level 20, each Knurl provides 16.0 HP/s instead of 8.0 HP/s. Multiple Rejuvenation Racks stack further.
Poorly. Transcendence converts all but 1 HP into regenerating shields. The +40 HP per stack adds almost nothing since only 1 HP remains. The regen also only applies to HP, not the shield pool. Knurl is best on builds that keep natural HP. If you are running Transcendence, prioritize other boss items.
