How the Frenzy Actually Works

In-game says: NO cooldowns
Actual: all skills snap to 0.5s minimum cooldown
The 0.5s floor prevents animation conflicts and engine crashes. Not truly zero - but near-zero.
Kill an elite, every skill recharges at 0.5s for the frenzy window. The moment the kill registers, all cooldowns instantly drop to 0.5s and stay there for the duration. Skills with 20-second cooldowns are treated identically to skills with 2-second cooldowns during the frenzy.
Stacks extend the window, not the power. 1 Brainstalks = 4s frenzy. 2 stacks = 8s. 3 stacks = 12s. The 0.5s cooldown floor never changes with stacks. More stacks mean more time to chain skills, not faster skills.
Chaining elite kills extends the buff. Each elite kill during an active frenzy refreshes the timer. On dense elite stages, a single Brainstalks can sustain near-permanent uptime through back-to-back kills.
Some skills still have animation locks. Loader's charged punch, Artificer's charged Nano-Bomb, and Bandit's gun spin animation all prevent other skills from firing while they play out. Brainstalks does not skip these. Skills that require holding a button (Artificer Flamethrower, Huntress Arrow Rain) benefit most since they can be reused immediately.
Forgive Me Please RoR2Forgive Me Please does not trigger Brainstalks. The doll fires on-kill effects but Brainstalks requires an actual elite kill by the player. The same applies to Wake of Vultures RoR2Wake of Vultures and Defense Nucleus - none of these are triggered by FMP dolls, even with Artifact of Honor active.

Flamethrower held through the entire frenzy window outputs enormous sustained damage. Nano-Bomb still has animation lock but Plasma Bolt becomes near-continuous. Often considered Artificer's best red item.
Commando RoR2
Commando A Tier
Suppressive Fire and Frag Grenades become spammable. Multiple grenades in one frenzy window can delete bosses by stacking simultaneous explosions before the damage registers.
Captain RoR2
Captain A Tier
Orbital Probe becomes spammable during frenzy. A 4-second window can deliver 6+ probes to a single target. One of the most reliable burst damage windows in the game.
Loader RoR2

Grapple Hook becomes chain-castable for rapid repositioning and proc chains. Charged Knuckle still has animation lock but Thunderslam becomes effectively spammable during the window.

Epidemic becomes spammable, potentially blanketing the stage with Blight stacks. However Acrid's damage relies more on passive DoT than skill spam, so extra casts hit diminishing returns faster than other survivors.

Key Synergies

Artifact of Honor is the strongest pairing. Every enemy spawns as an elite. Every kill triggers Brainstalks. With even 1 stack, frenzy windows chain continuously through combat. By mid-run the buff is effectively permanent as long as you are actively killing. This is the defining Brainstalks build.
Wake of Vultures RoR2Wake of Vultures fires on the same elite kill. One elite kill simultaneously grants Brainstalks frenzy AND the dead elite's passive power. With Artifact of Honor both stack constantly, giving near-permanent elite buffs (Celestine slow, Blazing burns) alongside near-permanent near-zero cooldowns.
Old Guillotine RoR2Old Guillotine extends frenzy windows. Guillotine executes elites below a HP threshold, generating more elite kills per engagement. More triggers mean more frequent Brainstalks windows. Both items scale identically with elite density and compound naturally.
Shatterspleen RoR2Harvester's Scythe RoR2Shatterspleen and Harvester's Scythe during frenzy. Spamming skills fires many more proc rolls per second. Shatterspleen bleeds stack per-hit during the frenzy window and can trigger mass explosions. Harvester's Scythe heals on every crit, providing massive sustain across the burst window.
Alien Head RoR2Alien Head, Hardlight Afterburner, and Purity become redundant. Brainstalks with Artifact of Honor replaces the need for any cooldown reduction item. Those item slots are better filled with damage or survivability. Without Artifact of Honor, CDR items still have value between frenzy windows.

Highly build-dependent. On Huntress and Artificer it is S-tier - those survivors have holdable or repeatable skills that scale directly with free cadence. On Engineer it is weak because turret limits and missile lock-ons don't benefit from cooldown reduction. With Artifact of Honor it becomes one of the best red items in the game regardless of survivor since the uptime becomes near-permanent.

How do you unlock Brainstalks?

Complete the Deicide challenge by killing an elite boss on Monsoon difficulty. The fastest method is to enable Artifact of Honor (which forces all enemies to spawn as elites, guaranteeing the Teleporter Boss is an elite) and clear Stage 1 on Monsoon. Once the kill registers, Brainstalks starts appearing in your item pool from the next run onward.
