How It Works

All HP except 1 becomes shields. Your health bar disappears and is replaced entirely by a shield bar. Every HP-increasing item you pick up after, and every level you gain, converts the extra HP directly into more shields.
Shield regen requires 7 seconds without any damage. Even a single tick of DoT resets the timer. Once 7 seconds pass, your shields regenerate fully over 2 seconds. Mobility and dodging are the core skill requirement for this item.
Fall damage near-immunity. Fall damage is calculated from max HP (1) and cannot reduce HP below 1. The worst any fall can do is deal 1 damage to your single HP. Your shields absorb no fall damage.
Percentage-based heals become useless. Bustling Fungus heals 4.5% of max HP per second. Your max HP is 1. That is 0.045 HP per second - effectively nothing. Strides of Heresy, Foreign Fruit, and all other % heals share this fate.
Flat heals fire but cannot restore shields. Leeching Seed, Harvester's Scythe, and Monster Tooth still trigger their heal amounts normally. But shields only regenerate naturally - healing items cannot directly top them up. Those heals are wasted unless you hold Aegis to convert them to barrier.
Healing Disabled debuff does nothing. Malachite elites apply Healing Disabled on hit, which blocks all healing. Since you're not relying on healing anyway, this debuff is irrelevant. Transcendence players are functionally immune to it.

Heretic has negative base HP regen that slowly kills her. Transcendence converts all HP to shields, which regenerate positively on a 7-second timer. The death spiral is completely eliminated. One of Heretic's most valuable items.
REX
REX - Run-Killer
REX spends HP to fire skills and heals from damage to sustain. With 1 HP, skill costs are near-lethal and healing cannot restore the HP-for-shields trade. Transcendence completely dismantles REX's kit synergy. Sacrifice immediately.
Eclipse Mode Synergies

Transcendence bypasses three Eclipse modifiers at once: Eclipse 1 (halved starting health) is ignored since all HP converts to a larger shield pool. Eclipse 3 (increased fall damage) is nearly negated by the 1 HP retention. Eclipse 5 (halved healing) does nothing since you're not using healing. This makes Transcendence one of the strongest Eclipse items.

It is a run-killer for REX (whose kit relies on self-damage and healing synergy). On Heretic it actually fixes the negative HP regen problem by converting all HP to positively-regenerating shields. In Eclipse mode it bypasses several penalties.

Does Transcendence work with Aegis?

Yes - Aegis is the best item to pair with Transcendence. Since you're always at "full HP" (your max 1 HP is always at 1), every flat heal fires directly into Aegis barrier. Leeching Seed, Harvester's Scythe, and similar flat healing items all generate barrier rather than being wasted. Note that percentage heals are still near-zero and don't help Aegis either.

Does Transcendence make you immune to fall damage?
