Holy Book is a Legendary item that does three things at once: it adds +100 Max HP, +50 HP Regen per second, and converts all your healing into a damaging aura. Every time you heal - from HP Regen ticks, Lifesteal, Medkit, Campfire, or any other healing source - that healing radiates damaging energy outward to nearby enemies.
This means Holy Book is simultaneously one of the best defensive items in the game and an offensive one. The more healing you generate per second, the more damage pulses you emit. In a high-regen build with Regen Tome, Medkit, and Campfire, Holy Book can produce a near-constant damage aura around your character that kills enemies passively while you focus on movement and positioning.
Holy Book's +50 HP Regen bonus is affected by Leeching Crystal's multiplier. If you have 2+ Leeching Crystals, Holy Book's regen contribution is zeroed out or turned negative. The healing damage aura requires active healing to trigger - if your net HP Regen is zero or negative, the aura does not pulse from regen ticks. Build Holy Book and Leeching Crystal together only if you have strong Lifesteal to compensate.
Holy Book is one of the strongest Legendary items in the game. The raw stat block alone - +100 Max HP and +50 HP Regen - is exceptional for a single item slot. The healing aura on top of that makes it a dual-purpose pickup that fits into virtually any build that plans to stay alive.
It is most impactful in builds that actively stack healing: Regen Tome, Medkit, Campfire, and Lifesteal all amplify the aura. In a fully stacked healing build, Holy Book can deal meaningful passive damage to every nearby enemy on every tick, effectively giving you a free area-of-effect weapon that scales with your defensive investment.
Even in builds that do not stack regen specifically, the +100 Max HP and +50 base regen alone justify taking it in most runs.
The "Holy Trinity" is a community-named build combination of Holy Book, Mirror, and Aura. Each item covers a different role: Holy Book provides sustain and passive healing damage, Mirror creates a clone that doubles your weapon output and provides invulnerability frames on damage, and Aura deals constant AoE damage to all nearby enemies. Together they create a self-sustaining build that can stand in enemy groups indefinitely while all three effects work simultaneously.
The Holy Trinity was most dominant before the Kevin + Mirror patch in v1.0.7, but remains a strong late-game setup for Tier 3 runs and Final Swarm survival.

The unlock requires a no-damage boss kill on any first Stage Boss - it does not need to be a specific map or tier. The easiest approach is to attempt this on Tier 1 Forest with a ranged build, keeping maximum distance from the boss throughout the fight. Characters with high mobility - Calcium or Ninja - make staying out of boss attack range more reliable. Shield Tomes do not count as taking damage, so a Shield-heavy build provides a safety net if you are struggling to maintain full HP.
The unlock condition requires no HP damage. Shields absorb hits before HP takes damage. Building a large shield with Shield Tome before the boss fight gives you several hits of buffer without technically taking HP damage, making the no-damage unlock significantly more forgiving.
Athena - The wiki directly notes Holy Book as great for Athena. Athena uses Aegis as her weapon, which has a cooldown-based playstyle. Holy Book's constant HP Regen keeps Athena's HP stable and the healing aura adds passive damage output that Aegis alone does not provide. The combination makes Athena one of the strongest sustained damage dealers in the game.
Megachad - Megachad's naturally high Max HP gets even larger with Holy Book's +100, and his Flex ability absorbs damage spikes that would otherwise interrupt the regen loop. The healing aura gives Megachad passive damage output during the large portions of each run spent soaking hits in enemy groups.
Vlad - Vlad's +1% Lifesteal per level passive means he is constantly healing from every hit. Holy Book converts that healing into a damage aura on every lifesteal tick, turning Vlad's already high healing rate into a passive offense tool. In a late-game Vlad build with high attack speed, the lifesteal procs and corresponding Holy Book aura pulses become near-continuous.
