How the Delivery Works

One Crashed Multishop spawns per player holding the item. In multiplayer, if 3 players each hold a Shipping Request Form, 3 separate deliveries spawn on every stage. Each player gets their own terminal to pick from.
The delivery is completely free. No gold cost, no resource expenditure. It is a burning Multishop Terminal with 2 item slots - pick one, the other closes. One slot always shows a question mark (unknown rarity until you approach).
Works even with Artifact of Sacrifice active. Artifact of Sacrifice When Sacrifice removes all chests from stages, Shipping Request Form deliveries still spawn. This makes it one of the only sources of item choice in Sacrifice runs.
Picking it up mid-stage delays the first delivery. If you find and pick up a Shipping Request Form during a stage, no delivery spawns that stage. The first Crashed Multishop appears when you enter the next stage.
Corrupts all Rusted Keys immediately. Every Rusted Key in your inventory converts to a Shipping Request Form on pickup. If you had 3 Rusted Keys, you now have 4 Shipping Request Forms (original + 3 converted).
Never offers equipment. Deliveries can roll Common, Uncommon, or Legendary items only. Equipment is explicitly excluded from the delivery pool regardless of stack count.

Legendary weight scales exponentially with stacks (0.01 x n²) while uncommon scales linearly (0.20 x n). In multiplayer, stacks from all players combine for rarity weighting.

Multiplayer Stacking - Spread the Item

In multiplayer all players' stacks combine for rarity weighting. If one player has 5 stacks and another has 1, BOTH deliveries use the rarity weight of 6 stacks. It is almost always better to spread Shipping Request Forms across multiple players - more free deliveries AND shared rarity boost.

Key Synergies

Executive Card Executive Card lets you take both items. Normally you pick one of the two delivery choices. With Executive Card you take both - effectively doubling the value of every delivery. In multiplayer one player with an Executive Card can open all teammates' deliveries and claim both items from each.
57 Leaf Clover 57 Leaf Clover boosts the hidden rarity roll. The question mark slot's rarity is rolled randomly. Clover's luck bonus improves all random rolls including this one, pushing the unknown slot toward higher rarities.
Artifact of Sacrifice Artifact of Sacrifice removes chests - but not deliveries. In a Sacrifice run, Shipping Request Form deliveries are one of the few remaining sources of guaranteed free item choice. Stack multiple for consistent quality loot when enemies are your only other option.

Tips

Pick it up early. The item only generates deliveries from the stage after you get it. An early pickup means more total deliveries across the run.
Spread it in multiplayer. Each player with the item gets their own delivery. With combined stacks boosting rarity, spreading Shipping Request Forms is almost always more valuable than one player hoarding all copies.
Check both delivery slots before picking. One slot shows the rarity, one is a question mark. Approach both to see what they are before committing. The question mark slot often reveals higher rarity items than expected at high stack counts.
Picking this up converts all your Rusted Keys. If you have multiple Rusted Keys already, consider whether the conversion is worth it. Getting 4 SRFs from 1 original + 3 keys is often excellent, but losing high-quality keys late game may not be.
