"Seems to do nothing... but..." Beads of Fealty is a lunar item with no combat effect whatsoever. Its only function: when any player holding it obliterates at the Obelisk in A Moment, Fractured, the entire team is transported to A Moment, Whole instead of ending the run. There you fight one of four Twisted Scavengers for 10 Lunar Coins. Stacking beyond one does nothing. It is the only item in the game with completely no stat effect of any kind.
A bleak hidden realm - the refuge of the Twisted Scavengers. One of four Twisted Scavengers waits at the center. Defeat it and loot its sack before the screen fully darkens to collect 10 Lunar Coins. Defeating the Scavenger counts as obliterating (completing mastery challenges on Monsoon). Dying does not.
One of the four is randomly selected each visit. All carry preset item and equipment loadouts - they are significantly stronger than regular Scavengers. Their sack drops 10 Lunar Coins on death.

Find and pick up Beads of Fealty during a run (from Lunar Pods or the Bazaar Between Time), then reach A Moment, Fractured via a Celestial Portal. Interact with the Obelisk and confirm obliteration. As long as at least one player holds Beads of Fealty, all players are transported to A Moment, Whole instead of ending the run.
Longstanding Solitude is a challenge completed by reaching A Moment, Whole via Beads of Fealty and completing the run there - which means defeating the Twisted Scavenger. Dying to the Twisted Scavenger does not complete it. The challenge rewards are not an item unlock but serve as a completion record.
True Respite is completed by obliterating yourself at the Obelisk for the first time. It unlocks the Mercenary survivor. You can complete it with or without Beads of Fealty - if you have Beads, you'll be sent to A Moment, Whole instead of ending, which also counts. If you don't have Beads, the run simply ends and True Respite is completed.
Yes. Beads of Fealty must be held by at least one player each time you want to reach A Moment, Whole. They are not a one-time unlock. Every run that uses this path requires at least one Beads of Fealty.
Stacking Beads of Fealty beyond the first has no additional effect. One is sufficient to redirect the obliteration to A Moment, Whole. Extra stacks are useful only as Cleansing Pool fodder since Beads have no downside and are safe to sacrifice.
